Job summary

Harmonyhomehealth is seeking a Weekend Admissions Nurse to facilitate the admission of new residents and deliver high-quality nursing care during the transition into our facility.

The RN will perform comprehensive assessments, collaborate with physicians and the care team, and ensure accurate documentation and regulatory compliance. Excellent communication with residents and families is essential for a smooth admission experience.

Qualifications

• Current RN license in the state of practice (Utah)
• Strong assessment and critical thinking to determine care needs
• Ability to collaborate with physicians and other health team members
• Excellent communication with residents, families, and staff

Responsibilities

• Conduct thorough admissions assessments for new residents and document health history
• Develop individualized care plans with the care team
• Communicate admissions process and facility policies to residents and families
• Complete admission paperwork and collect required records
• Coordinate with dietary, therapy, and other departments for services
• Administer medications as prescribed
• Monitor residents' vital signs during admission and report changes
• Provide emotional support to residents and families during admission
• Ensure regulatory compliance for admission processes and resident rights
• Participate in team meetings and discharge planning as needed
• Perform additional duties as assigned
