Job summary

Advanced Health Care of Glendale is seeking an RN Admissions Nurse to facilitate the admission process for new residents and provide high-quality nursing care during transitions. This full-time role includes weekend shifts (Fri, Sat, Sun, 11AM–11PM) and requires collaboration with physicians and the care team to create individualized plans.

You will conduct comprehensive assessments, complete paperwork, administer medications as prescribed, monitor vital signs, and support residents and families

Qualifications

• Valid RN license in the state where work is performed.
• Valid CPR card.
• Experience as admission nurse or intake coordinator in a healthcare setting preferred.
• Strong assessment and critical thinking skills.
• Knowledge of regulatory requirements governing admissions.
• Ability to build rapport with patients through compassion.
• Excellent interpersonal and communication skills.
• Proficient in basic computer software and electronic medical record systems.
• AZ Fingerprint Card and AZ TB Test.

Responsibilities

• Conduct thorough assessments of incoming residents to gather medical history and assess care needs.
• Collaborate with physicians and staff to develop individualized care plans.
• Communicate admissions process, policies, and services to residents and families.
• Complete admission paperwork and obtain required documentation.
• Coordinate with dietary and therapy for necessary assessments and services.
• Administer medications and treatments as prescribed.
• Monitor residents' vital signs and report changes to the care team.
• Provide emotional support to residents and families during admission.
• Ensure compliance with regulatory requirements related to admissions and resident rights.
• Participate in team meetings and discharge planning as needed.
• Perform other duties assigned by the Director of Nursing.
