Job summary

Horizon Home Care & Hospice is looking for a Home Care Admission Nurse to coordinate client admissions while ensuring the highest quality care. Responsibilities include assessing conditions and fulfilling educational needs of patients and families.

Qualifications

• Degree from an Accredited School of Nursing (BSN preferred).
• Current WI RN license.
• 1-2 years of Med/Surg or Home Care experience.
• Current/Valid WI Driver's License.

Responsibilities

• Assess patient and family conditions accurately during each visit.
• Identify and provide for patient's educational needs.
• Complete all required patient paperwork accurately and timely.
• Communicate patient's issues to Case Manager and Clinical Manager.
