Job summary

Triangle seeks a Direct Care worker for a part-time role at Green Street, providing essential support to individuals served. The position covers Saturday and Sunday shifts from 10am to 6pm, totaling 16 hours per weekend.

Responsibilities include direct care, documentation, activity support, and collaboration with the Residential Program Manager to promote growth and well-being of individuals. Training and supervision are provided as scheduled.

Qualifications

• High school diploma or GED with 1–3 months related experience or training, or an equivalent combination of education and experience.

Responsibilities

• Provide direct care coverage and support for individuals served during assigned shifts.
• Assist individuals in working on areas identified in their Individual Support Plans.
• Complete documentation including communication logs, case logs, progression notes, incident reports, and medication records.
• Transport and accompany individuals to appointments and community activities as needed.
• Attend supervision and staff meetings and participate in trainings as directed.
