Job summary

Triangle is seeking Direct Care staff to provide direct care and support for individuals served at Sherwood Ave. shifts include Sunday 10a-8p, Monday 3p-11p, and Saturday 5p-11p, totaling 24 hours per week.

You will assist with daily living activities, document progress, and collaborate with the Program Manager to implement Individual Support Plans. Requirements include a high school diploma or GED and 1–3 months of related experience or training.

Qualifications

• High school diploma or GED required or equivalent.
• 1–3 months related experience or training.
• Ability to verbally communicate, read, analyze, interpret, and write documents.

Responsibilities

• Provide direct care coverage and support during assigned shifts.
• Assist individuals with growth in quality of life areas.
• Complete documentation such as progress notes and incident reports.
• Work with supervisor on behavior support plans and safety procedures.
