Job summary

Triangle is seeking a Direct Care staff member to provide direct support for individuals served at our Lawrence Street location. The role includes responding to issues during scheduled shifts and promoting growth across key quality of life areas.

The position requires a high school diploma or GED and emphasizes teamwork, documentation, and adherence to Triangle policies. Weekend hours are Sunday evenings and Saturday day shifts, with supervision provided by the Residential Program Manager.

Qualifications

• Ability to verbally communicate, read, analyze, interpret, and write documents.

Responsibilities

• Provide direct care coverage and support for individuals served, responding to individual issues during assigned shifts.
• Work with individuals to promote growth in quality of life areas: control, respect, dignity, relationships, community membership, personal growth & accomplishments, and personal well-being.
• Assist individuals in working on areas identified in their Individual Support Plans with team collaboration.
• Support individuals in problem solving, personal care, medical care, socialization, and activities of daily living.
• Assist in development and implementation of behavior support plans.
• Know emergency procedures and basic emergency facts for all individuals at the location.
• Complete documentation such as logs, charts, progress notes, incident reports, and medication charts.
• If certified to dispense medications, report occurrences and follow procedures.
• Transport individuals to appointments and community activities as needed.
• Attend supervision and staff meetings and trainings as scheduled.
