Job summary

Concierge Home Care is seeking a Weekend PRN Intake Specialist to support patient access to home health services. This remote role processes referrals, verifies patient and insurance information, and collaborates with hospitals, providers, Sales, and Operations to ensure a smooth transition into care.

You will work Saturdays and Sundays, 8:30 AM–5:00 PM, from a home workspace within 30 minutes of a Concierge Home Care branch. Florida residency is required and ongoing training is provided.

Qualifications

• Minimum 1 year experience in medical intake, referrals, or healthcare administration.
• Experience navigating EMR systems and handling referrals.
• Experience with insurance verification and eligibility checks.
• Florida residency required; reliable remote workspace and internet.

Responsibilities

• Receive and process incoming patient referrals from hospitals and providers.
• Verify patient demographics and insurance information; collect required documentation.
• Navigate hospital and portal systems to process referrals and track status.
• Collaborate with internal teams to ensure timely referral processing and follow-up.
• Maintain HIPAA-compliant handling of patient information and documentation.
