Hays Nursing & Rehabilitation Center in San Marcos, TX, is seeking a weekend PRN Occupational Therapist to join our in-house rehab program. You will work with a geriatric patient population, delivering rehabilitative care and documenting progress.

Responsibilities include screening, assessments, ADL retraining, therapeutic activities, and communicating with the care team. Open to all experience levels; licensure required; weekend shifts provide flexibility in a post-acute setting.

Qualifications

• Licensed Occupational Therapist with active licensure in the state.

Responsibilities

• Screen/evaluate patients with cognitive and ADL function disorders and develop plans of care.
• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative treatment per regulatory and clinical requirements.
• Perform ADL retraining, therapeutic activities, exercises, modalities and device use.
• Monitor progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed based on results.
• Counsel patients and families about physical disorders and care paths.
• Communicate progress and plans with supervisor and health team.
• Provide comprehensive discharge summaries per regulatory requirements.
