Job description

Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

Position: Full-time Physical Therapist (PT) for weekend home health visits in Charlotte, NC.

Territories Covered: Cabarrus, Gaston, Iredell, Mecklenburg, and Union counties.

Availability: Must be available Fri-Sun or Sat-Mon.

Requirements: Home Health experience is REQUIRED.

Responsibilities:

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and update care plans, collaborating with nursing staff.
6. Educate clients and families on rehabilitative activities.
7. Document all observations and interventions accurately using electronic medical records.

Qualifications:

1. Current North Carolina PT license.
2. Graduation from an accredited physical therapy program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with electronic medical records; prior EMR experience preferred.
