Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time, weekend home health visit position at our senior living office in Charlotte, NC. The role involves providing care across territories in Cabarrus, Gaston, Iredell, Mecklenburg, and Union counties.

Availability: Must be available Fri-Sun or Sat-Mon.

Home Health Experience REQUIRED!

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative and skilled nursing care to help clients remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
4. Develop and execute individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and update care plans, collaborating with nursing staff as needed.
6. Educate clients and family members on rehabilitative care and safety activities.
7. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications:

1. Current North Carolina Physical Therapist (PT) license.
2. Graduation from an accredited program approved by the American Physical Therapy Association or equivalent.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; prior EMR experience preferred.
