Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time weekend home health opportunity in Charlotte, NC.

This office services adult and geriatric clients across Cabarrus, Gaston, Iredell, Mecklenburg, and Union counties.

Availability required: Fri-Sun or Sat-Mon

Home Health Experience REQUIRED!

As a home care Physical Therapist, you will be a key member of a multidisciplinary team providing rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include:

• Making home visits in designated territories.
• Performing diagnostic tests, such as mobility, strength, balance, and safety assessments.
• Implementing wound care interventions using evidence-based dressings and topical agents.
• Developing and updating individualized care plans, including manual therapy, gait training, and balance exercises.
• Assessing and revising care plans regularly and collaborating with nursing staff.
• Educating clients and families on rehabilitative activities to promote health and independence.
• Documenting care activities accurately using a touch pad tablet.

Qualifications:

• Current North Carolina PT license.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
