Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem weekend opportunity providing home health visits for our Charlotte office, located in Charlotte, NC. This office serves adult and geriatric clients across Mecklenburg & Union counties.

REQUIREMENT: HOME HEALTH & OASIS DOCUMENTATION EXPERIENCE

Availability: 2-4 weekends per month (Saturday & Sunday)

As a home care Physical Therapist, you will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

1. Make home visits in designated areas.
2. Perform diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
3. Implement wound care interventions with evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and update care plans, collaborating with nursing staff.
6. Educate clients and families on rehabilitative activities to promote safety and independence.
7. Document observations and interventions accurately using electronic tablets.

Qualifications:

1. Current North Carolina Physical Therapist license.
2. At least one year of recent PT experience.
3. Graduation from an accredited program approved by the American Physical Therapy Association.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal and organizational skills.
6. Proficiency with computers; EMR experience preferred.
