Job summary

Responsibilities

• Establishes and implements a plan of care; monitors outcomes and adjusts treatment as needed.
• Coordinates discharge planning with patient education and home program instructions.
• Completes documentation, ensures billing and scheduling accuracy, and adheres to department policies.
