Job summary

You will monitor progress, document care, and coordinate with physicians, ensuring safe, effective discharge planning and family education. Travel between site locations may be required.

Qualifications

• DPT/physical therapy degree from accredited program.
• Licensed Physical Therapist in the state of practice (IL).
• Acute care experience preferred.

Responsibilities

• Evaluate patients and establish care plans.
• Implement treatment and adjust plan based on progress.
• Discharge planning and patient/family education.
• Document and chart patient care and outcomes per policy.
