Job summary
St. Mary Rehabilitation Hospital is seeking a Pharmacist to provide essential weekend coverage every other Saturday and Sunday. You will work within a small interdisciplinary team, independently managing weekend pharmacy operations after onboarding.
Ideal candidates have hospital pharmacy experience and a PharmD or BS Pharm with licensure, with onboarding support provided to ensure confidence and preparation for weekend duties.
Qualifications
- Graduate of an accredited school of pharmacy; BS Pharm or PharmD required; accredited clinical residency or BCPS certification preferred.
- Current state license to practice pharmacy.
Responsibilities
- Verify physician medication orders with accuracy and timeliness; clarify and resolve discrepancies.
- Monitor and evaluate medication management processes (selection, storage, ordering/transcribing, preparation/dispensing, administration) for quality and safety.
- Verify and approve work by Pharmacy Technicians; provide coaching and feedback.
- Order, receive, and store medications to maintain integrity and compliance.
- Review patient-specific drug therapy for indications, dosing, interactions, and contraindications; document interventions.
- Perform anticoagulant monitoring and communicate dose/therapy recommendations per protocol or provider order.
