Job summary

St. Mary Rehabilitation Hospital is seeking a Pharmacist for weekend PRN coverage at our 50-bed inpatient rehab facility. You will verify medication orders, manage pharmacy operations, and collaborate with a small interdisciplinary team.

Prior hospital pharmacy experience preferred; BS Pharm or PharmD required; current state licensure. After training, you will independently handle weekend duties with autonomy.

Qualifications

• Education: BS Pharm or PharmD from an accredited school.
• Licensure: Current state license to practice pharmacy.
• Experience: Two years hospital pharmacy experience preferred.

Responsibilities

• Verify physician medication orders with accuracy and timeliness.
• Monitor and evaluate medication management processes and drive quality improvements.
• Review patient-specific drug therapy for indications, dosing, interactions, and contraindications.
• Perform anticoagulant monitoring and dose recommendations per protocol.
