The role requires bilingual Cantonese, an active California RN license, and experience in acute care, home health, or hospice. Schedule includes Tue/Fri 3:30 pm–12:00 am and Sat/Sun 8:30 am–5:00 pm, with competitive pay and benefits.

Qualifications

  • Bachelor's or associate degree in nursing required, CA RN license.
  • Experience in acute care, home health, or hospice preferred.

Responsibilities

  • Provide continuing nursing care to patients and families per plan of care.
  • Under supervision of After-Hours Nurse, assess, plan, implement and evaluate nursing services.
  • Coordinate with physicians and community resources for optimal patient care.
  • Maintain communication with Team Leaders regarding patient status.

Our spectrum of home‑based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. We are seeking an After‑Hours Nurse who will provide skilled nursing and hospice care to maximize the comfort and health of patients.

Must be bilingual in Cantonese.

Schedule:

Tuesday & Friday; 3:30 pm – 12:00 am

Saturday & Sunday; 8:30 am – 5:00 pm

Location:

San Francisco County with coverage as needed to Marin County

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities (but are not limited to):
  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Under the guidance of the After‑Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
  • Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
  • Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
Education / Experience:
  • Associate's or Bachelor’s degree in nursing from an accredited School of Nursing.
  • Experience in acute care / home health and/or hospice setting preferred.
Certificates, Licenses, Registrations:
  • Current license as a registered professional nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.
Pay Range:

Pay Range: $64.50 – $78.70 shift differential is included.
