Job summary

Mission Hospice & Home Care is seeking a per diem After-Hours Nurse to provide skilled nursing and hospice care to maximize patient comfort and health. The role covers weekends, Saturdays and Sundays 4:30pm–12:30am, in San Mateo, CA, serving San Mateo County.

Requirements include one year of professional nursing experience in the past three years or a BSN, active California RN license, and preferred hospice or home health experience.

Qualifications

• One year of nursing experience in the past three years or a Bachelor’s degree in Nursing.
• Experience in acute care, home health, and/or hospice is preferred.
• Current California RN license.
• Public Health Nursing certification and HPNA certification are desirable.

Responsibilities

• Provide continuing care to patients and families per agency policies and applicable regulations.
• Under after-hours supervision, assess, plan, implement, and evaluate nursing services per the plan of care.
• Communicate with attending physicians and community resources to coordinate care.
• Maintain regular communication with Team Leaders regarding patient/family status.

Schedule

Every Saturday & Sunday; 4:30pm - 12:30am

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
• Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range

$52.90 - $66.20 hourly + $2.00 weekend differential
