Job summary
UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes within the LA service area. You will work under the supervision of a Registered Nurse and coordinate care with the health team.
Responsibilities include documenting visits, following care plans, and ensuring high-quality patient care while adhering to state regulations. Compensation includes hourly pay, benefits, and opportunities for career growth.
Qualifications
- Current and unrestricted LPN licensure in state of practice.
- Current CPR certification or ability to complete within 90 days of hire.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Responsibilities
- Provides high quality clinical nursing services in scope of practice per plan of care and infection control procedures.
- Documents patient visits in the EMR per regulatory and payer requirements.
- Coordinates care with health team and communicates patient needs to relevant parties.
