Job summary

CenterWell Home Health is hiring a Patient Care Coordinator in North Carolina. The role requires an active, unrestricted NC RN or LPN license and offers a part-time schedule with a 9-hour shift on Saturdays and Sundays.

Qualifications

• RN/LPN license required in North Carolina.
• Bachelor’s degree in Nursing preferred.
• 3 years of clinical experience preferred.
• Knowledge of governmental regulations and Medicare eligibility requirements.
• Excellent customer service, organization, and communication skills.
• TB screening requirement if selected for role.

Responsibilities

• Develop and maintain contact with hospitals, SNFs, assisted living discharge planners to update market services.
• Collaborate with program staff and field sales resources.
• Assist in processing physician orders.
• Report to a Market Executive of Sales.
