Job summary

CenterWell Home Health is seeking a Patient Care Coordinator in North Carolina. This role requires an active, unrestricted NC nursing license and entails developing relationships with discharge planners, collaborating with program staff, and assisting with physician orders.

The position is part-time with a 9-hour shift on Saturdays and Sundays, and candidates must live within an hour of Delco, NC. You will report to a Market Executive of Sales and attend weekly sales meetings as required.

Qualifications

• RN/LPN license in NC required.
• BSN preferred.
• 3 years clinical experience preferred.
• Knowledge of governmental regulations, Medicare eligibility, and care plan coordination.
• Excellent customer service, account development, organization, problem-solving and communication skills.
• This role is part of Humana's TB screening program and may require TB screening.

Responsibilities

• Develop and maintain contact with hospital, SNF, and discharge planning services to provide updates on services.
• Collaborate with program staff and other field sales resources.
• Assist program in processing physician orders.
• Report to a Market Executive of Sales.
