Job summary

UnitedHealth Group is recruiting a Part-Time RN Case Manager for weekend days to oversee the Plan of Care for hospice patients in Louisiana. You will evaluate patient needs, coordinate care with the IDG, and ensure high-quality, compassionate service consistent with our mission.

The role requires current RN licensure, a valid driver’s license, and the ability to work with diverse patient families. Prior hospice or palliative care experience is preferred.

Qualifications

• Must hold current RN license in the state of practice.
• Must have a valid driver's license and dependable vehicle or access to public transportation.
• Preferred: 1+ years clinical experience; CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patient/family needs and re‑assesses at least every 15 days.
• Initiates and coordinates the plan of care; documents goals, interventions and responses.
• Collaborates with patient/family, attending physician and IDG for daily care.
• Instructs and supervises patient/family in self‑care techniques when appropriate.
• Maintains accurate clinical notes and ensures care quality.
