Job summary

Trilogy Health Services, LLC is seeking an LPN for a weekend 7pm-7am shift at our Avon, Indiana location. You will provide direct patient care, supervise nursing assistants, monitor residents’ status, and ensure accurate documentation.

The role requires an active Indiana LPN license and CPR certification, with comprehensive Thrive benefits and supportive team culture. This position offers weekend-only scheduling with competitive pay.

Qualifications

• Current, valid LPN license in Indiana.
• Current CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify charge nurse or physician when there is a change in condition.
• Identify resident problems and emergency situations and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure an adequate supply is on hand.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting as necessary.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies.
