Job summary

Trilogy Health Services, LLC in Fort Wayne is seeking a Weekend Supervisor (LPN or RN) to assist the Director of Nursing and ADON in guiding weekend nursing operations, ensuring the highest quality of resident care and adherence to care plans. Active LPN or RN license and CPR certification are required.

Twelve-hour shifts occur on weekends with the Director of Health Services’ schedule, plus a weekend incentive of $7.00 per hour for hours Fri–Sun.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Current, valid CPR certification required.

Responsibilities

• Assist the ADON & DON to plan, develop, organize, implement, evaluate, and direct the nursing service department.
• Supervise, oversee and assist with all resident care according to assessed resident status and plan of care.
• Provide direct nursing care as necessary per each resident’s assessment, physician orders and plan of care.
• Assist in scanning and attaching resident documentation in MatrixCare to ensure Medical Records are complete and accurate.
• Maintain first impressions to welcome potential customers to experience our services and participate in survey and mock-survey campus preparedness.
• Ensure campus is tour ready including back up team resources, tour path, model room, first impressions, and tour stations.
