Job summary

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans including socialization and health maintenance.
• Assist with daily activities such as personal care, housekeeping, transportation, and shopping.
• Document progress, maintain confidentiality, and follow care plans.
• Monitor health and report health concerns to supervisor.
• Uphold safety and rights of individuals in all activities.

Education

High school diploma or equivalent preferred

Six months of experience in human services preferred

Must be 18 years of age

Working knowledge of computers
