Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, transportation, shopping.
• Maintain records and confidentiality; review logs for changes in service plans.
• Provide safe, respectful care and promote independence and community inclusion.
