Job summary

Hartford HealthCare at Home is seeking an On‑Call Visiting Nurse to provide high‑quality, person‑centered care to clients in their homes or community settings. You will support a long‑standing network delivering home‑care services in Connecticut, with on‑call shifts including Friday 4:30 pm–Saturday 8 am and Saturday 4:30 pm–Sunday 8 am.

Responsibilities include on‑call duties, conducting nursing assessments, collaborating with physicians and case managers, and guiding LPNs and Home Care Aides.

Qualifications

• One year of nursing experience or one year in a specialty such as medical/surgery or home care.
• Licensed to practice as Registered Nurse in Connecticut.
• Proficient computer skills for documentation and communication.

Responsibilities

• Assume responsibility for evening, night, weekend, and holiday on-call duty as scheduled.
• Manage the region census during the assigned shift, including assessment, planning, and evaluation.
• Maintain regular communication with Nurse Supervisors, attending physicians, and case managers.
• Respond to medical or emotional crises and perform home visits to provide direct care.
• Review referrals and initiate contact with clients and families.
• Conduct complete nursing assessments based on referral data and clinical observation.
