Job summary

Pathwell Health is seeking an on-call Home Health RN in Connecticut. The role centers on weekend coverage from Friday evening to Monday morning with admission and as-needed visits, plus ongoing nursing care and case management for patients at home.

The ideal candidate holds an active Connecticut RN license, is CPR certified, and has home health experience. Excellent communication, organizational, and bilingual (Spanish) skills are a plus; weekend availability is required.

Qualifications

• Weekend availability Friday 4:30 pm–Mon 8:00 am ET
• Associate degree in nursing; BSN preferred
• Current Connecticut RN license
• CPR certification current
• Driver’s license, car insurance & registration
• Home health care experience preferred
• Bilingual (Spanish) preferred
• Strong communication and reliable interpersonal skills

Responsibilities

• Provide skilled nursing care as ordered by the physician
• Complete evals with vital signs and med reviews
• Coordinate with physicians, therapists and care team
• Assess and chart client condition at each visit
• Maintain accurate nursing records
• Educate patients and families on home health care
• Record medical information and vitals
• Monitor, report symptoms or changes in condition
• Administer applicable medications or therapies as prescribed
• Prepare patients for exams or treatments
• Adapt care plans as needed
• Order, interpret and evaluate diagnostic tests
• Engage in research and ongoing training
• Perform other duties as assigned
