Job summary

Hartford HealthCare at Home is seeking a Revisit Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes and community settings, under RN supervision. Weekend shifts include Sa, Su, Monday, with emphasis on patient education and outcome-based care.

You will document progress in EMR, collaborate with the care team, and participate in ongoing quality improvement. CT LPN licensure and a valid driver's license are required.

Qualifications

• High School diploma or GED required.
• Complete CT state approved LPN certificate program.
• Experience in nursing care required.
• Proficiency with Outlook, Word and Excel.

Responsibilities

• Provide intermittent skilled nursing services under RN direction.
• Support patient/family using teaching principles to promote health.
• Record daily progress in EMR.
• Report to primary nurse, supervisor, physician, social worker or therapist.
• Communicate with primary nurse and other disciplines.
• Consult with physician regarding client needs and plan of care.
• Assist in evaluation of care provided by paraprofessionals.
• Participate in agency committees and special projects.
