Nurses At Home is seeking a Weekend Registered Nurse (RN) to join our Greater Hartford/Farmington Valley home health team in Connecticut. The role offers weekend coverage with scheduling flexibility and potential to earn additional visits on weekends.

The RN will provide skilled home nursing, SOC/ROC assessments, medication management, wound care, and disease monitoring while coordinating with physicians and the care team.

Qualifikationen

• Active Connecticut RN license is required.
• Minimum of 1 year nursing experience.
• Home health experience strongly preferred.
• Strong assessment and clinical skills are essential.
• Excellent communication and documentation abilities.
• Ability to work independently in the field.
• Reliable transportation and a valid driver’s license.

Aufgaben

• Provide skilled nursing visits in patients' homes.
• Conduct Start of Care (SOC) assessments.
• Perform Resumption of Care (ROC) visits.
• Manage medications and educate patients.
• Perform wound care as needed.
• Monitor and manage diseases and conditions.
• Communicate with physicians and care team members.
• Complete timely, accurate clinical documentation.

Nurses At Home is looking for an experienced and dependable Weekend Registered Nurse (RN) to join our growing home health team.

We are a locally owned home health agency that has grown while maintaining the small, family feel that makes our team different. Our nurses have the ability to work independently in the field while still having strong clinical and administrative support when they need it.

The Position

The Weekend RN will provide skilled nursing care to patients in their homes, including:

• Skilled nursing visits

• Start of Care (SOC) assessments

• Resumption of Care (ROC) visits

• Medication management and patient education

• Wound care

• Disease management and monitoring

• Communication with physicians and other members of the care team

• Timely and accurate clinical documentation

Schedule

This position is focused on weekend coverage, with scheduling flexibility based on patient needs and nurse availability.

This is a great opportunity for an RN looking to earn additional income on the weekends without committing to a traditional full-time schedule.
