Visiting Nurse & Hospice of Litchfield County is seeking a Weekend RN to deliver home health visits on Saturdays and Sundays in Litchfield County. Hours are 8:30am–4.30pm, with pay for 20 hours in a 15-hour schedule.

The role includes weekend On-Call duties (4:30pm–8:30am) in the absence of the On-Call RN, requiring CT licensure, home health/hospice experience, and familiarity with OASIS and HOPE assessments. NetSmart knowledge is a plus.

Competenze

• Must hold a current CT RN license in good standing.
• Experience in home health or hospice required.
• Experience with OASIS and HOPE assessments preferred.
• Knowledge of NetSmart software is a plus.

Mansioni

• Provide weekend home health visits in Litchfield County on Saturdays and Sundays.
• Support weekend On-Call duties (4:30pm-8:30am) when needed.
• Coordinate patient care with the hospice team and families.

Visiting Nurse & Hospice of Litchfield County is looking for a Weekend RN, to provide home health visits in Litchfield County on Saturdays and Sundays, 15 hours, get paid for 20 hours. 8:30am-4:30pm shift. Candidate will also assist with Weekend On-Call duties (4:30pm-8:30am) in the absence of weekend On-Call RN.

Must have current Connecticut RN license in good standing.

Must have home health/hospice experience.

Must have experience with OASIS and HOPE assessments

Knowledge of NetSmart software a plus.
