Job summary

Trinity Health Group LLC is seeking an On-Call Registered Nurse (RN) for weekend day coverage. The role supports hospice patients and families after hours with timely triage, home visits, and careful documentation to ensure comfort and quality of life.

The nurse will work Saturdays and Sundays 7 AM–7 PM, providing compassionate, patient-centered care and collaborating with the hospice team to maintain continuity of care.

Qualifications

• Current, active RN license in the State of Texas.

Job Summary:
We are seeking a dedicated and compassionate On-Call RN to join our hospice team. The ideal candidate will provide after-hours support to hospice patients and families, responding promptly to calls, triaging needs, and making home visits as required to ensure the highest quality of care.

Responsibilities:

• Provide on-call hospice nursing support during night hours.
• Conduct home visits for urgent patient needs, symptom management, or pronouncement of death.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care.

Qualifications:

• Current, active RN license in the State of Texas.
• Minimum of one year of nursing experience (hospice or palliative care preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communication and patient advocacy skills.
• Reliable transportation auto insurance and valid driver’s license.
