Job summary

LilyCare Hospice in central New Mexico seeks a Weekend On Call Hospice Registered Nurse (PRN) to cover patients on weekends. You will respond to after-hours calls, provide in-home symptom and pain management, and perform admissions and pronouncements as needed during on-call hours.

Qualifications

• Active NM RN license in good standing (compact/multistate accepted).
• Current AHA BLS/CPR certification.
• Hospice or home care experience preferred; comfortable working autonomously on call.

Responsibilities

• Respond to after-hours calls and triage patient and family needs during weekend on-call.
• Make in-home visits for symptom and pain management and crisis response.
• Complete hospice admissions and pronouncements during on-call hours.
• Coordinate care and communicate changes with the interdisciplinary team and on-call physician.
• Document calls and visits in the electronic health record per hospice requirements.
• Provide a clear handoff to the weekday team at end of coverage.
