Job summary

LilyCare Hospice is seeking a per-visit Hospice Registered Nurse On Call PRN to care for patients across your New Mexico territory. You will provide skilled nursing visits in patients' homes and facilities, assess symptoms, and manage comfort and pain at end of life.

Qualifications

• Active NM RN license in good standing (compact accepted).
• Current BLS/CPR certification and willingness to maintain certification.
• One year of RN experience; hospice or home care strongly preferred.
• Valid NM driver’s license and a reliable, insured vehicle for patient visits.
• Strong assessment, end-of-life symptom management, and thorough EHR documentation.

Responsibilities

• Conduct skilled nursing visits in patients' homes and facilities, assessing symptoms, pain, and comfort needs.
• Develop, implement, and revise the plan of care with the interdisciplinary team and attending physician.
• Administer medications and provide expert pain and symptom management for end-of-life patients.
• Educate and emotionally support patients and families throughout the hospice journey.
• Document every visit accurately and on time in the electronic health record, meeting visit and regulatory requirements.
