Job summary

Del Corazon Hospice seeks a PRN Hospice RN to provide patient-centered end-of-life care in collaboration with the interdisciplinary team. This per diem role offers on-call coverage and flexible scheduling to meet patient and team needs.

Responsibilities include pain and symptom management, patient education, and detailed communication with physicians, families, and caregivers. A NM RN license, BCLS, and valid driver’s license are required, with hospice or home health experience preferred.

Qualifications

• RN licensure in NM (current)
• 1 year of relevant RN experience in last 2 years (acute, home health, community health, or hospice)
• Current BCLS certification
• TB testing or ability to obtain on day one and background check passed
• Valid NM driver’s license and reliable vehicle with proof of insurance

Responsibilities

• Provide patient-centered care and symptom management.
• Collaborate with IDT to develop and implement care plans.
• Triage calls and make visits as clinically warranted.
• Pronounce expired patients when necessary.
• Maintain thorough documentation and communicate with team.

At Del Corazon Hospice, our mission is to provide high quality, compassionate care to patients during their end of life journey. We are committed to providing exceptional care to our patients and their families, and we are seeking a dedicated and compassionate PRN Hospice RN to join our team.

Job Summary:

As a PRN Hospice RN, you will provide high-quality, patient-centered care to individuals with serious illnesses. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, manage pain and symptoms, and provide emotional and spiritual support to patients and their families. This is a per diem position, allowing for flexibility and autonomy in your scheduling.

Responsibilities:

• After hours on-call; primary responsibility
• Triage calls and make visits as clinically warranted
• Pronounce expired patients
• Collaborate with IDT post on-call to give accurate and detailed report
• Flat rate for on-call + PRN visit rate for each visit made.
• Collaborate with patients, families, and interdisciplinary team members to ensure coordinated care
• Educate patients and families on disease processes, treatment options, and self-care techniques
• Document patient care and communicate with team members, physicians, and other healthcare professionals

Requirements:

• Graduate of an accredited school of nursing.
• Current Registered Nurse licensure in the state of New Mexico
• Experience as an RN commensurate with one (1) of the following:
  • One (1) year acute care experience within the last two (2) years
  • One (1) year home health care experience within the last two (2) years
  • One (1) year community health or public health experience within the last two (2) years
  • One (1) year hospice experience within the last two (2) years
  • Current BCLS certification.
  • Current TB testing (or able to receive on day one) and passes a criminal background check.
  • Valid state driver’s license and reliable automobile.
  • Proof of current automobile insurance.
• Hospice experience preferred.
