Job summary

Titan Placement Group is seeking a Weekend On-Call Hospice Case Manager (RN) in Lakeville, MA. This weekend-only role offers on-call nursing support to hospice patients in homes and facilities within Easton, Lakeville, Taunton, and Rockland.

You’ll provide visits and admissions as needed, collaborate with the interdisciplinary team, and manage symptoms while traveling in the field. Massachusetts RN license and 1+ year of nursing experience are required.

Qualifications

• Active Massachusetts RN license
• At least 1 year of nursing experience
• Hospice, home health, ICU, or oncology experience preferred.

Responsibilities

• Provide on-call nursing support to hospice patients in homes and facilities
• Complete visits and admissions as needed
• Collaborate with the interdisciplinary hospice team to ensure continuity of care
• Assess patient condition changes and provide symptom-management support
• Respond independently to patient needs in the field
• Travel across Easton, Lakeville, Taunton, and Rockland
