Job summary

Titan Placement Group is seeking a Weekend On-Call Hospice Case Manager (RN) in Lakeville, MA. This weekend-only role offers on-call nursing support to hospice patients in homes and facilities within Easton, Lakeville, Taunton, and Rockland.

You’ll provide visits and admissions as needed, collaborate with the interdisciplinary team, and manage symptoms while traveling in the field. Massachusetts RN license and 1+ year of nursing experience are required.

Qualifications

• Active Massachusetts RN license
• At least 1 year of nursing experience
• Hospice, home health, ICU, or oncology experience preferred.

Responsibilities

We’re hiring a Weekend On-Call Hospice Case Manager (RN) who wants the flexibility of a weekend-only role while providing compassionate end-of-life care to hospice patients and their families.

You’ll provide on-call nursing support to hospice patients in homes and care facilities throughout Easton, Lakeville, Taunton, and Rockland. You’ll respond to patient and family needs, complete visits and admissions as needed, and collaborate with the interdisciplinary hospice team to ensure continuity of care.
