Qualifications

• Registered nurse with strong clinical assessment and critical thinking skills.
• Experience with home health care, wound care, and IV therapy preferred.
• Ability to triage by phone and handle urgent situations professionally.
• Strong communication skills for patient/family education and team coordination.
• Comfort with EHR documentation and care coordination.

Responsibilities

• Perform nursing assessments during urgent and routine home visits.
• Conduct phone triage and respond to urgent patient needs.
• Manage medications and coordinate care plans.
• Provide wound care and administer/monitor IV therapies.
• Coordinate with physicians and the interdisciplinary care team.
• Document all care and observations in the EHR.
