Job summary

Pathwell Health is seeking a registered nurse to provide on-call home health services in California. The RN will cover weekend on-call from Friday 4:30 pm to Monday 8:00 am, assess patients, develop and implement care plans, and coordinate with the care team.

Responsibilities include supervising visits, documenting medical records, conducting health maintenance guidance, and managing admissions visits as needed. California RN license and CPR are required; bilingual Spanish is a plus.

Qualifications

• Weekend on-call availability Friday 4:30 pm to Monday 8:00 am.
• California RN license and CPR certification required.
• Home health experience preferred; bilingual Spanish preferred.

Responsibilities

• Provide skilled nursing care per physician orders.
• Assess patient health problems and develop care plans.
• Maintain medical records and document nursing care.
• Educate clients and families on home health procedures.
• Coordinate with physicians and other care team members.
