Qualifications

• Master’s or doctoral degree from an accredited nurse practitioner program.
• Active, unrestricted Virginia Nurse Practitioner license.
• Current DEA registration, as required for the role.
• National board certification in the relevant population focus.
• Experience in post-acute care, long-term care, geriatrics or related clinical setting preferred.
• Proficiency with EMR systems and strong clinical documentation skills.

Responsibilities

• Provide high-quality, patient-centered medical care in post-acute care settings.
• Assess patients’ health needs, develop care plans and monitor clinical progress.
• Order and interpret diagnostic tests, prescribe treatments and medications, and make referrals as appropriate.
• Collaborate with nursing staff, physicians, APNs and other team members to support outcomes.
• Communicate health concerns and treatment plans with patients, families and care teams.
• Document assessments and follow-up plans accurately in EMR.
• Support care coordination and transitions between healthcare settings.
