Job summary

Legacy Care’s independent medical group seeks a skilled Nurse Practitioner for a full-time role in Richmond, VA. You will deliver patient-centered primary and post-acute care across skilled nursing, long-term care and assisted living settings, with autonomy and collaborative support.

Responsibilities include assessments, diagnosis, treatment planning, prescription management and care coordination with interdisciplinary teams to ensure safe, effective continuity of care.

Qualifications

• Master’s degree or DNP from an accredited program.
• Active Virginia NP license and eligibility to practice in Virginia.
• Current national NP board certification.
• DEA registration and prescriptive authority as required.
• Experience in primary or post-acute care settings preferred.
• Strong communication, collaboration and documentation skills.

Responsibilities

• Provide comprehensive primary and post-acute medical care in skilled nursing and related settings.
• Conduct initial and follow-up evaluations, review history and current concerns.
• Diagnose and manage acute and chronic conditions; create treatment plans.
• Prescribe and manage medications in line with regulations and guidelines.
• Monitor outcomes, adjust care, and coordinate with care teams.
• Document accurately in the EMR and participate in care planning meetings.
