Job summary

Legacy Care seeks an experienced Nurse Practitioner to support our Richmond team on a PRN basis. You will provide primary and post-acute care across skilled nursing, long-term care and assisted living settings, with independent judgment and collaboration with physicians and staff.

Qualifications

• Master’s degree or DNP/NP from an accredited program.
• Current Virginia NP license and eligibility to practice.
• National NP certification.
• DEA registration and prescriptive authority.

Responsibilities

• Provide comprehensive primary and post-acute medical care in skilled nursing and long-term care settings.
• Assess, diagnose and manage acute and chronic conditions.
• Prescribe and manage medications per clinical guidelines.
• Coordinate care with multidisciplinary teams and facility staff.
• Maintain accurate EMR documentation and ensure patient safety.
