Job summary

Legacy Care is seeking a dedicated Nurse Practitioner to provide patient-centered care in post-acute settings in Richmond, Virginia. This part-time opportunity offers meaningful contributions while working with a supportive multidisciplinary team.

You will assess and manage patients, collaborate with staff, and support continuity of care across the patient journey. The role offers flexible scheduling coordinated with facility coverage needs and credentialing support where applicable.

Qualifications

• Master’s or doctoral degree from an accredited NP program.
• Active, unrestricted Virginia Nurse Practitioner license.
• Current DEA registration, as required for the role.
• National board certification in the relevant population focus.
• Experience in post-acute care, geriatrics or a related clinical setting is preferred.
• Proficiency with EMR systems and strong clinical documentation skills.
• Strong clinical judgement, communication and interpersonal skills.
• Ability to work autonomously while collaborating effectively with an interdisciplinary team.
• A compassionate, patient-centered approach to care.

Responsibilities

• Provide high-quality, patient-centered medical care in post-acute care settings.
• Assess patients’ health needs, develop care plans and monitor clinical progress.
• Order and interpret diagnostic tests, prescribe treatments and medications, and make referrals as appropriate to your scope of practice.
• Collaborate with nursing staff, physicians, advanced practice providers and other members of the multidisciplinary team to support positive patient outcomes.
• Communicate effectively with patients, families and care teams about health concerns and treatment plans.
• Use electronic medical record (EMR) systems to document assessments, care provided and follow-up plans accurately and promptly.
• Support care coordination and transitions between healthcare settings as appropriate.
