Impact Health PC in Lansdale, PA is seeking an experienced NP or PA to provide weekend clinical coverage, Saturdays and Sundays, with potential for one or both days. Qualifications

• Active NP or PA license.
• Minimum 2 years of post-certification experience.
• Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred.
• Proficient with EHRs and clinical technology.
• Strong communicator with a collaborative mindset.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams.
• Conduct assessments, develop treatment plans, and manage follow-up care.
• Educate patients, families, and staff.
• Complete timely documentation (within 24 hours).
• Ensure compliance with state regulations in SNF settings.
