Job summary

Aviata Health Group seeks a Nurse Practitioner (Skilled Nursing) to lead internal medicine services at a Skilled Nursing Facility on Saturdays and Sundays. You will collaborate with an interdisciplinary team and physicians to reduce weekend hospitalizations, serving short- and long-term residents with nursing staff support.

Qualifications

• Advanced Practice Licensure (APN) with a Florida license.
• Must have an active license and DEA number.
• Previous experience in nursing or other medical fields.
• Ability to build rapport with patients and loved ones.
• Ability to thrive in a fast-paced environment.
• Excellent written and verbal communication skills.
• Strong leadership experience with teamwork in a healthcare setting.
• Master's degree required.

Responsibilities

• Evaluation of new-admission visits.
• Coordination of discharge visits.
• Evaluations for patients with changes in condition.
• Provide comprehensive health assessments and differential diagnoses.
• Prescribe treatments under protocol and manage acute/chronic illness.
• Perform comprehensive physical examinations.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and facilities.
• Communicate with attending physicians and specialists regarding patient care.
