Aviata Health Group is seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a SNF setting, where you will serve as a clinical leader coordinating with the facility’s care team to oversee admissions, discharges, and changes in residents’ conditions.

You will handle advanced care planning, comprehensive assessments, and prescribing treatments, contributing to optimal resident outcomes and care continuity.

Qualifications

• Active Florida APN licensure and DEA license required.
• Prior nursing or clinical experience in SNF or related settings.
• Master’s degree in Nursing is required.
• Strong communication and teamwork skills are essential.

Responsibilities

• Oversee admissions and discharges for SNF residents.
• Conduct monthly stability checks and evaluations for changes in condition.
• Lead advanced care planning and comprehensive assessments.
• Prescribe treatments as part of the care plan.
