Job summary

Aviata Health Group is seeking a Nurse Practitioner (Skilled Nursing) to provide internal medicine services at a Skilled Nursing Facility on Saturdays and Sundays. You will work with the facility’s team to reduce hospitalizations and manage a diverse resident population under physician supervision.

The role requires an active Florida license, APN licensure, and a DEA number, with a Master’s degree and prior nursing/medical experience.

Qualifications

• APN licensure for Florida is required.
• Active Florida license and a DEA number are mandatory.
• Previous nursing or medical field experience is expected.

Responsibilities

• Act as Skilled Nursing Facility’s lead internal medicine clinician on Saturdays and Sundays.
• Collaborate with interdisciplinary team and physicians to reduce weekend hospitilizations.
• Care for short- and long-term SNF residents with nursing and medical guidance.
• Evaluate new admissions and coordinate discharge visits.
• Assess patients with changes in condition and adjust care plans.
• Provide comprehensive health assessments and differential diagnoses.
• Prescribe treatments under protocol for acute and chronic illnesses.
• Conduct comprehensive physical examinations and implement management policies.
• Facilitate referrals to other health professionals and facilities.
• Communicate with attending physicians and specialists regarding patient care.
