Job summary

Aviata is seeking a Nurse Practitioner to serve as a leading internal medicine provider at a Skilled Nursing Facility on Saturdays and Sundays in Florida. You will collaborate with the facility staff and physicians to reduce hospitalizations, evaluate new admissions, and manage discharges for short- and long-term residents.

You will deliver comprehensive health assessments, develop care plans, and prescribe treatments under protocol, while communicating with attending physicians and providing

Qualifications

• Advanced Practice Licensure (APN) with a Florida license.
• Must have an active license and DEA number.
• Previous experience in nursing or other medical fields.
• Ability to build rapport with patients and families.
• Ability to thrive in a fast-paced environment.
• Excellent written and verbal communication skills.
• Strong leadership experience with teamwork in healthcare.
• Master’s degree required.

Responsibilities

• Act as a Skilled Nursing Facility’s leading internal medicine provider on weekends.
• Collaborate with interdisciplinary team and physicians to reduce hospitalizations on weekends.
• Work with short and long-term residents with nursing staff guidance.
• Evaluate new admission visits.
• Coordinate discharge visits.
• Evaluate patients with changes in condition.
• Provide care including Advanced Care Planning and comprehensive assessments.
• Prescribe treatments under facility protocols and manage acute and chronic illness.
• Perform comprehensive physical examinations.
• Develop patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and facilities.
• Communicate with attending physicians and specialists regarding care.
