Job summary

Willow Creek Health Care Center in the United States is seeking an experienced LVN Admissions Nurse for a weekends-only schedule. The role encompasses completing admissions assessments, reviewing medical records, and coordinating with the admissions team to ensure a smooth transition into skilled nursing care.

The position offers pay ranging from $28.00 to $30.00+ per hour based on experience, with part-time weekend shifts and a strong emphasis on compassionate resident and family education.

Qualifications

• Active California LVN license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing, long-term care, or post-acute settings preferred.
• Admissions or charge nurse experience is a plus.

Responsibilities

• Complete nursing assessments for newly admitted residents.
• Review hospital records, discharge summaries, medication lists and orders prior to admission.
• Verify admission orders are complete and entered into the chart.
• Perform medication reconciliation and communicate discrepancies to physicians or supervisors.
• Initiate resident care plans and timely clinical documentation.
• Maintain accurate documentation in the electronic health record.
• Coordinate with admissions, dietary, therapy, social services, and MDS teams.
• Provide education to residents and families about the admission process and facility services.
