Job summary

Willow Creek Health Care Center seeks an experienced LVN Admissions Nurse to welcome new residents and ensure a smooth transition into skilled nursing care on weekends. This role collaborates with nursing leadership, admissions, physicians, residents, families, and the interdisciplinary team to complete admissions accurately and with excellent customer service.

The position offers competitive pay and benefits, with a weekends-only, part-time schedule and pay range of $28.00–$30.00+ per hour,

Qualifications

• Active LVN license in California in good standing.
• CPR/BLS certification is required.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical assessment and documentation skills.
• Experience with electronic health records (EHR) preferred.

Responsibilities

• Complete nursing assessments for newly admitted residents.
• Perform admission documentation and medication reconciliation.
• Coordinate with admissions dept and nursing leadership.
• Communicate with hospitals, physicians, families and other providers.
• Maintain accurate EHR documentation and care plans.
• Uphold HIPAA and safety policies.
